Unit Name: Nursing Practice in Aged Care & Rehabilitation Settings

Description: This Course describes the skills and knowledge required by Enrolled Nurses to perform nursing interventions to assist the person requiring rehabilitation to regain optimal function and lifestyle. Work as part of the multidisciplinary health care team for the client requiring rehabilitation.

Learning Target Outcomes: At the completion of this course, the student should be able to: 1. Identify physical and psychological effects of ageing on the human body and apply to provision of information and support. 2. Apply physical and psychosocial assessment when evaluating health of older people. 3. Use nursing care plan according to needs of ADL when patients’ circumstances change. 4. Identify rehabilitation principles that underpin nursing interventions for the older person and apply to nursing care. 5. Identify effects of grief and loss on physical, emotional, cognitive, behavioural, social, and spiritual domains. 6. Document outcomes and communicate to the registered nurse using a collaborative approach for follow up with multidisciplinary health care team. 7. Provide safe, holistic, patient-centered care to promote wellness across the lifespan in Rehabilitation hospital. 8. Interact with members of the healthcare team in meeting the needs of patients in a rehabilitative setting. 9. Demonstrate skills in basic nursing procedures.
