Unit Name: Hospital Care in the Home
Description: World Health Organization (WHO) has explained Universal health coverage as all people and communities getting access to promotive, preventive, curative, rehabilitative and palliative health services they need, of sufficient quality to be effective, while also ensuring that the use of these services does not expose the user to financial hardship. The course introduces the learner to the principle of Universal Health care and the concept of continuing hospital care in home settings. Components of home-based care is explored, and nursing models and theorists are covered to explain therapeutic environment in home settings. Continuing hospital care in the home discusses screening of eligible clients and looks at issues such as environmental hazards, infection control measures and personal threats before clients can be taken care of in home settings. Health care personals responsible for continuing client care in family settings must be an authorized practitioner working within their recognized scope of practice. Client Informed consent and ethico- legal issues are covered with regards to case management. The course will conclude with effective use of Nursing process which encompasses appropriate client and family education that will provide the client and caregivers with strategies and tools to manage care in the home setting, that will enable positive patient outcomes.
