Unit Name: Nursing Chronic Conditions in Family
Description: This course is designed to provide upskilling for community health nurses enabling them to view management of long-term conditions for clients in family settings. The course introduces you to various Global strategies that discusses measures aimed at controlling Non communicable diseases(NCD). Bio-psychosocial factors are explored to determine how they determine lifestyle factors in prevention and control of chronic conditions. Pathophysiology and pharmacology of common chronic conditions would be discussed, and client centered nursing care plans would be developed using the instruments of the nursing process. Principles of Health education and Motivational counselling would be discussed for lifestyle modification. You will also learn smoking cessation techniques and the course concludes by explaining the use of Chronic Care Model for management of clients with Long Term Conditions (LTCs’).
